Webster County Health Issue Survey
Fordland Clinic is attempting to obtain Federal funds to expand health services in Webster County.  This would include dental, mental health and expanded family practice and urgent care at the clinic available under a discounted fee schedule and funding for outreach activities (example: a dental hygienist to go to Head Start, day care’s and the school systems to both teach prevention/dental health and to do screenings and provide basic services).  We need community assistance to select the 10 areas that will be the focus areas for the first 3 years.

Please print the following survey.  Select 10 from the list as the problems you feel are most important AND that you can envision that something can be done to improve.  We ask that you send us an email simply listing the letters of the items in the order you feel are most important.  E-mail the results to RobertMarshNP@FordlandClinic.com

a.  Cancer rate (age adjusted death rate for all cancers per 100,000   people)
b.  Low birthweight rate (per 1,000 live births).
c.  Infant mortality rate (per 1,000 live births).
d.  Rate of exposure (i.e., hypothermia, heat stroke, etc.)
e.  Asthma rate.
f.   Diabetes rate.
g.  Nutrition rate (e.g. hunger, malnutrition, etc.)
h.  Coronary heart disease rate (age adjusted death rate for all CAD per 100,000).
i.   Rate of occupational (e.g., pesticide) and environmental hazard exposures.
j.   Skin Disorders rate (e.g., acne, eczema, ring worm, etc.)
k.  Dental disease rate (e.g., caries, lack of teeth, periodontal disease)
l.   Teen pregnancy rate.
m. Late entry into prenatal care.
n.  Obesity rate.
o.  Depression rate.
p.  Suicide rate.
q.  Hypertension rate.
r.   HIV/AIDS/STD's rate
s.  Unintentional injury rate
t.   Percent of elderly
u.  Substance abuse rate
v.   ADD/ADHD rate
w.  Rate of respiratory infection.
x.   Immunization rate
y.   Rate of school absenteeism
z.   Special issues of minority populations (Amish, black, Indian, Spanish, etc.)
aa. Rate of serious mental illness (e.g., schizophrenia, bi-polar, etc.)
bb. Other (must be health related) WE CAN HAVE ONLY ONE 'OTHER' write it in if you have something you would like to see addressed.
